If you choose to submit the inquiry form, we collect only what you enter:

• First and last name
• Email address
• Phone number (optional)
• Your relationship to the program (for example, family member or service coordinator) and reason for inquiry
• Any general message you choose to include (optional)
• The date and time of the submission

Please do not include medical information, diagnoses, Social Security numbers, or other sensitive personal information. The form does not ask for it, and general inquiries do not require it.

To follow up on your interest in the program and, if you ask, to share program information as it becomes available. We do not sell inquiry information or share it for third-party marketing.

Submissions are sent from your browser over an encrypted (HTTPS) connection and checked on our server before being saved to a cloud database operated for this website. The database is not publicly readable: visitors cannot view submissions, and the form itself can only add new entries.

Only individually designated administrators, signed in with their own account and granted administrator access, can view or delete submissions. Access is not given automatically to everyone who works on the website. To confirm: Name or role of the designated inquiry administrator

The website relies on the following providers, who process data on our behalf to run it:

• Lovable — website building and hosting platform, including Lovable Cloud database hosting.
• Supabase — database and account-sign-in infrastructure used by Lovable Cloud.
• Cloudflare — content delivery and hosting infrastructure for the website.

Automated email notifications are not currently in use. If they are enabled, the email delivery provider will be listed here. To confirm: Confirm provider list before launch

This website does not use advertising cookies, marketing pixels, or third-party tracking scripts. Like most websites, our hosting provider may automatically record standard technical information (such as IP address, browser type, and pages requested) for security and basic, aggregated visit statistics. Administrators who sign in have their session stored in their own browser's local storage. If analytics are added, this section will be updated first.

A retention schedule has not yet been decided. It will be set by Quantum Adult Day leadership with legal guidance and published here before the form opens to the public. To confirm: Retention period — leadership/legal decision

You may ask what information we hold from your inquiry, ask us to correct it, or ask us to delete it, subject to applicable law. Designated administrators can delete individual submissions.
